What bone head things have you done to ruin shooting on a dive?

I'm not sure what's scarier - the idea of not getting photos on a dive, or scraping my bare knees on some coral! :)

Well the water temp is 84 to 86 now! Don't want to get to dressed up!

So...ever leave the cap on?
Forget to connect the hot-shoe/flash?
Forget zoom gear?
Forget to switch lens [Nikon] to auto-focus?
